No. Coverage to Smile About has no deductibles, no yearly maximums, no claim forms, and no pre-authorization forms. You can use it right away.
Will a pre-existing condition or age keep me out?
No. There are no pre-existing condition limitations and no age limits. You and your family can have immediate eligibility.
What does the plan cover for cleanings and exams?
Diagnostic and x-rays are covered at 100%, including a comprehensive exam for new patients and periodic exams twice a year. Preventive cleanings and fluoride are also covered at 100%.
